General Information
Our office hours are Monday through Friday from 9:00 a.m.
to 5:00 p.m. Patient appointments may be scheduled any time
during office hours. It is important to bring your insurance
card with you to your appointment, as well as all medical
information, including x-rays, labs, medical history,
medications, etc. Prescription refills are best handled
through your pharmacy. Call in advance to avoid running out
during weekends or holidays. Please be prepared to provide a
urine sample when you arrive at your appointment.
We contract with many HMO and PPO organizations. These
organizations are always changing. Please be sure we are
able to provide your care prior to your visit. If a referral
is needed, be certain you have one that is current. We
expect payment for office services at the time of your
visit. MasterCard and Visa are accepted.
